La historia se centra en un accidente ferroviario en la localidad de Peacock, Nebraska. Dicho accidente, aparentemente fortuito, en realidad entraña un misterio que afecta a John, un empleado de banca (Cillian Murphy), quien prefiere pasar desapercibido por la vida. Pero John tiene un secreto: posee una personalidad oculta que nadie conoce; una mujer que cada mañana, antes de que John despierte, realiza las tareas domésticas y le prepara el desayuno. Hasta que un día todo cambia.

El filme es un drama bastante denso desde su trastocada atmósfera de desequilibrio psicológico, que a ratos puede llegar a interesar por cómo un mismo sujeto actúa diametralmente opuesto con sus dos personalidades, ocasionando numerosos conflictos dentro del contexto social en el que se desenvuelve.
